【Case Discussion】CHAPTER 4

Basic information: The male patient, aged 71, is a retired cadre and widowed.

Chief complaint: Difficult defecation for three months.

Medical history: The patient had difficult defecation three months before hospitalization. The stools were hard and dry and chestnut-like. The frequency of passing stools decreased dramatically, to 1-2 times per week. There was no abdominal pain, abdominal swelling, hematochezia, or mucous stools. He treated himself with Phenolphthalein Tablets, but the symptoms did not improve. He then sought medical care at the hospital.

Nursing evaluation: T36.8℃, P60 beats/min, R18 breaths/min, BP123/80mmHg. Body height was 170cm, and bodyweight was 85kg. There was a mass palpable in the left lower abdomen without tenderness. The number of bowel sounds was 3 per min, and percussion over the abdomen produced a dull note. He was physically healthy before the onset. History of intestinal surgery was denied. A recent physical examination indicated an abnormal increase in the blood lipid level and fatty liver. No other abnormalities were found. As to diet, this patient prefers meat and does not like vegetables. He also prefers spicy and stimulating food. Before retirement, he was once a bureau-level cadre. All his children live in other parts of the country. Now he lives alone. When he was young, he was a short-distance runner at the work unit. After retirement, he rarely takes other activities except for shopping. He spends most of the daytime reading books and newspapers or watching TV at home. He sometimes takes a nap on the sofa. He has difficulty falling asleep in the nighttime. He often wakes up in the middle of the night, and at other times he needs to take medication to fall asleep.

1. What may be the reasons for difficult defecation in this patient?

2. What can the nurse do to help treat difficult defecation?

3. For this patient, what are the precautions for physical exercises after discharge? How to guide the patient in physical exercises?
