这节课要讲的内容是脊髓
脊髓位于椎管内
自枕骨大孔后缘向后伸延至荐部
是中枢神经系统内的低级反射中枢
能直接完成一些低级的反射
同时也是脑和躯体相联系的中继站
脊髓的外形呈白色
背腹向稍扁的长圆柱状
依据与其联系的神经进出椎管的部位
可将其划分为颈髓 胸髓 腰髓 荐髓和尾髓
脊髓的外形有两个膨大部
位于颈髓后部和胸髓前部的称颈膨大
由其发出的脊神经分布于前肢
位于腰荐髓间的称腰膨大
发出的脊神经分布于骨盆腔及后肢
腰膨大之后的脊髓逐渐变细
形成圆锥状称为脊髓圆锥
自脊髓圆锥向后的细丝称为终丝
附着于尾椎椎体的背侧
有固定脊髓的作用
在脊髓圆锥和终丝的周围
被荐神经和尾神经包围
此结构总称马尾状的马尾
是临床上脊髓穿刺的安全部位
在脊髓表面还有背正中沟
腹正中裂 背外侧沟和腹外侧沟
它们是脊神经和血管出入脊髓的部位
脊髓的内部结构是
在中央有细长纵走的中央管
中央管的周围是灰质
灰质的外面是白质
灰质在脊髓的横断面上呈蝴蝶形
其背侧的突起称背侧角
腹侧的突起称腹侧角
在胸髓和腰髓段灰质的外侧
腹角的基部有较浅的隆起称外侧角
背侧角 外侧角和腹侧角的全长形成纵柱
分别称为背侧柱 外侧柱和腹侧柱
在中央管周围连接左右侧的
灰质称为灰质联合
脊髓灰质是由大量的神经元胞体
和少量的神经纤维
以及神经胶质细胞构成的
在背侧柱中主要是中间神经元的胞体
腹侧柱内侧的为运动神经元的胞体
胸腰段脊髓外侧柱
为交感神经节前神经元的胞体
荐段脊髓的中间外侧柱内
为副交感神经节前神经元的胞体
根据灰质内细胞形态和功能的差异
一般可以将其划分为十个板层
白质呈亮白色 位于脊髓表面
包围着灰质的周围
根据位置的不同 灰质可划分背侧索
外侧索和腹侧索
背侧索为背侧正中沟
至背外侧沟之间的白质
外侧索为背外侧沟
与腹外侧沟之间的白质
腹侧索为腹外侧沟
至腹正中裂之间的白质
白质主要为神经纤维构成
为脊髓上下传导冲动的传导径路
临床上背侧索的损坏会导致感觉丧失
腹外侧索的损伤可导致
感觉和运动障碍及某些肌肉麻痹
如动物椎间盘突出症就有类似症状
由于脊髓有分节的特性
临床症状能提供有关
脊髓损伤部位的信息
脊髓的主要功能
一是通过上下行传导束 传导神经冲动
二是通过脊髓内部的神经元联系
完成反射 如腱反射等
脊膜 脊髓外面包裹的三层结缔组织膜
总称为脊膜 由内向外依次为脊软膜
脊蛛网膜和脊硬膜
脊软膜薄 内含丰富的血管和神经纤维
紧贴于脊髓的表面
并突入至背正中沟和腹正中裂
脊蛛网膜薄而透明
与软膜之间形成较大的腔隙
称为蛛网膜下腔
该腔向前与脑蛛网膜下腔相通
内含脑脊液 以营养脊髓
在腰荐间隙或荐骨与第一尾椎间
空隙处的蛛网膜下腔 间隙增大
可作为临床上抽取脑脊液
或注射药物的部位
脊硬膜是白色致密的结缔组织膜
与脊蛛网膜之间形成狭窄的硬膜下腔
内含淋巴液向前方与脑硬膜下腔相通
在脊硬膜与椎管之间有一较宽的腔隙
称为硬膜外腔 内含静脉和大量脂肪
有脊神经通过
在临床上做硬膜外麻醉时
即通过在最后腰椎与第一荐椎间的
腰荐间隙将麻醉药注入硬膜外腔
可阻滞脊神经的传导作用
