我们先来看这幅漫画
它反映的是压床
目前
我国较多养老院只能提供养老而无法医疗
而医院只能医疗而不能提供养老服务
这种医养分离的结果
是由于养老院无法提供专业化的康复护理服务
造成许多老人将医院当成了养老院
即使病治好了
也要占着床位不出院
形成严重的压床现象
而一些慢病的老年人
由于担心养老机构的专业医护人员短缺
看病不方便等
而不愿意入住养老院
则造成养老床位空置现象
那么
有没有既能养老又能看病的地方呢
这就是我们今天要介绍的
医养结合养老照顾模式
医养结合养老照顾模式
是指将医疗资源与养老资源相结合
养老机构和医院功能相结合
集医疗 护理 康复 养生 养老于一体
实现社会资源利用的最大化
为老年人提供生活照料和医疗
康复 护理服务的新型养老照顾模式
医养结合具有以下基本特点
从保障目的来看
与传统养老模式一样
医养结合旨在为老年人提供老年生活服务
使老人安度晚年
从参与主体来看
它联合传统养老机构与医疗机构
旨在通过多元化的参与主体
为老年人提供一种新型的养老服务
从服务内容来看
由于引入了现代医疗技术
它能够提供更加专业
便捷的养老服务
有效提高老年人的晚年生活质量
从保障对象来看
尤其适宜处于大病康复期
慢性病 易复发病患者
等无法在传统养老模式中
得到良好照料的失能 半失能老人
从人性角度来看
它同时考虑了老年人的养老需求与医疗需求
符合现代老年人医养共需的基本生活需求
从广义范畴来界定
医养结合不仅是将传统养老保障
与现代医疗有机结合的一种新型养老方式探索
还意味着一种跨越式的养老新理念
医养结合与传统养老模式有何不同呢

首先
与传统养老模式相比
两者提供服务内容的差异最为明显
医养结合模式在提供传统养老模式
所包含的养老服务的基础上
为老年人提供专业化医疗和康复服务
实现了传统养老服务与现代养老服务的结合
其次
医养结合模式不同于
传统养老模式具有明确的责任主体
比如居家养老的责任主体是家庭
机构养老的责任主体是
各类型养老机构
医养结合没有明确的责任归属主体
在具体实践中
开展医养结合养老服务
可以是设有老年病科的医疗机构
或者是医疗机构分设
下属的养老服务单位
也可以是和医疗机构开展合作的
养老院 福利院
因此
医养结合并不是
作为一种独立的养老模式而存在
更多意义上
是作为一种新型的养老服务供给方式而运转
它可以和任何传统养老模式相结合
在任何养老模式中
以不同形式实现医养结合的服务供给
医养结合的类型包括
以医为主 以养为主 医养并重三种
医养结合项目中
以医为主的项目
通常以医院为依托
面向老人提供专业照护功能
面向人群是
以失能 半失能 失智等
老年人群为主
部分长期卧床
需要全面照护老年人为辅
医的需求是
急性医疗 专业护理 康复训练
养的需求包括
全面的生活护理 临终关怀
常见类型是
综合医院开设老年病科
康复科
医院附属的护理机构
医院改制的康复机构等等
医养结合项目中
以养为主的模式
通常以养老社区 公寓为依托
配套二级及以下医院
满足社区老人日常看病和急救的需求
面向人群包括
自理型老人为主
部分需要护理的老人

针对自理型老人的健康管理
慢病管理 康复运动
到针对老人的专业护理以及急救
养的需求包括
生活服务 娱乐服务

自理型社区加护理型公寓加专科医院
举个例子
重庆青杠老年护养中心就是医养并重型养老院
它是全国首家医院下属的养老机构
是由重庆医科大学附属第一医院全资兴建
是2009年国家发改委下达的
基本养老服务体系建设试点项目
是全国第一家由大型公立医院主办
并正式运营的养老机构
设置养老床位3000张
医疗床位1000张
由普通护养区 临湖护养区
临湖疗养楼 学术交流中心
康复医院 护理职业学院等组成
青杠老年养护中心设置了慢病区和护理院
主要针对需要后期的康复训练和专业护理
通过与综合医院的功能互补
帮助减少了
综合医院病人长期压床的压力
缩短了平均住院时间
也降低了病人的住院费用
同时也解决了养老机构老人医疗救治的问题
弥补了下级医疗单位
在处理复杂病症时能力不足的问题
从重庆青杠老年护养中心的例子我们可以看出
医养结合中的 医
主要包含三个内容
一 是急救医疗
可以在养老项目中设置医疗室
设置急救设施或是120急救车
与医院合作开通急救通道
让老人在身体出现异样时得到及时的救助和治疗
二 是健康管理
也是医养结合服务模式的核心价值所在
针对老年慢性疾病进行健康管理
三 是康复和护理
以养老机构为主体
对老年人进行康复锻炼指导和生活护理指导
为什么这些年国家屡次下发医养结合模式的

它可以有效整合现有的医疗和养老资源
拓展养老机构的功能

医疗保健 康复护理 文化娱乐等服务
