又名先天性幽门狭窄
课程内容包括
临床表现 流行病学 病因和诊断治疗
先天性幽门狭窄多发于3到6周的新生儿
表现为餐后喷射性呕吐
因为脱水会造成低氯性代谢性碱中毒
器质性的异常发生在幽门肌
渐进性肥大造成了幽门狭窄甚至是阻塞
幽门肌渐进性肥大的示意图如下
先天性幽门狭窄的流行病学有以下特点
其一发病率较高
西方国家千分之2至5
高于亚洲国家的万分之六
其二 有性别偏差
男女比例是4到5比1
其三 用瓶哺法喂养的孩子的
幽门狭窄发病率高于用母乳喂养的孩子
其四 出生顺序对发病率有影响
第一胎的孩子
比后续出生的孩子的发病率要高
先天性幽门狭窄的病因不明
遗传模式有复杂疾病的特征
即环境因素与遗传因素共同作用的结果
环境诱因有酒精
比如 胎儿酒精综合症
还有具胃动素激动剂功效的红霉素
遗传因素包括三方面
多基因遗传 单基因遗传和染色体缺陷
孤立型的先天性幽门狭窄
遵从多基因遗传模式
一 家族性聚集
二 同卵双生发病率200倍于普通人群
而异卵双生发生率为20倍于普通人群
它的遗传率高达87%
第三 致病基因的功能多样
所编码蛋白之间的相关性不强
综合症型的先天性幽门狭窄
遵从单基因遗传模式
目前发现54种综合症
出现了幽门狭窄的症状
其致病基因也导致
神经肌肉结缔组织失常
代谢失常 细胞信号和交流紊乱
纤毛病 DNA修复紊乱 转录调控失常
MARK通路失常 淋巴系统异常
此外已知有16种染色体缺陷
会导致先天性幽门狭窄
包括母源单亲二倍体
缺失 转位 重复 非整倍体
三体以及嵌合单体
最后谈谈先天性幽门狭窄的诊断和手术治疗
超声波是检测先天性幽门狭窄的高敏感性
高特异性的手段

先天性幽门狭窄需要手术切除
狭窄梗阻段
手术前需要做足充分准备
针对体液和电解质的流失的新生儿
进行复苏补偿葡萄糖 氯化钾
矫正血清中氯化钠和碳酸氢钠的水平
有不同的手术方法治疗幽门狭窄
这两张图片展示的是Ramstedt发明的
幽门肌切割术
另外还有腹腔镜幽门肌切割术
如图所示
手术后密切注意防范术后并发症
严重并发症的发生率为1.2%到3.4%
包括不完全幽门肌切割 肠粘膜穿孔
伤口感染 术后呕吐造成的延迟进食
浆膜撕裂和皮下气肿
轻微并发症发生率为7.3到23.8%。
综上所述
幽门肌肥大造成的消化道梗阻
为新生儿呕吐营养不良的常见病
男性比女性高发病因仍不明朗
遗传和环境都有影响
超声波诊断幽门肌切割手术治疗
预防和及时处理并发症
是手术成功和恢复的关键
