寒热的第四种类型就是寒热往来
寒热往来的特点就是恶寒和发热交替出现
这是正邪相争互为进退的病理反应
通常为半表半里证
临床上常见以下面两种类型
第一种是寒热往来无定时
是指 病人自觉时冷时热
一天内多次发热没有规律性的症状
通常见于少阳病 为半表半里证
因为外感病邪进入半表半里的阶段 既不在表也不在里
正邪相争 邪不盛而正气也不是很强
正胜则发热 邪胜则恶寒
所以恶寒和发热交替出现 发无定时
寒热往来第二种类型就是 寒热往来有定时
这是指的病人恶寒和发热交替出现
每天或者两三天发作一次 发作有规律的症状
通常伴有剧烈的头痛
口渴 多汗的症状
这种恶寒发热有定时
通常见于疟疾
主要是疟邪
潜伏于半表半里的膜原的部位
入则与阴相争则寒
出与阳相争则热
所以
恶寒和发热也会交替出现 休作有时
此外
寒热往来
亦见于气郁化火的证候中
临床应当根据
病史及其他兼症详细辨识
