本节课主要介绍产前超声检查的分级和时机
产前超声检查的分级
产前超声检查分为一 二 三 四 四个等级
一级超声主要针对胚胎和胎儿
进行大致的生长发育评估
检查内容仅要求进行双顶径
股骨长径及腹围的测量
判断胎儿是否存活
胎盘位置 胎位以及羊水情况等的检查
该级别的超声检查
不是以检测胎儿畸形为目的
但对产科临床仍能提供一些有意义的诊断信息
适合条件较差的基层医院
或者条件较好的医院
已经进行过系统超声检查的孕妇
二级超声除要求完成一般产科超声检查内容外
还应对胎儿主要脏器进行形态学观察
比如颅内某些重要结构
四腔心切面 腹腔内肝 胃 肾等脏器
二级超声要求对胎儿严重致死性畸形
进行粗略的筛查
卫生部《产前诊断技术管理办法》中规定
于妊娠18到24周
应诊断的致死性畸形包括无脑儿
严重的脑膨出 严重的开放性脊柱裂
严重胸及腹壁缺损
内脏外翻 单腔心 致死性软骨发育不全
三级超声通过对胎儿解剖结构进行系统筛查
达到提高胎儿畸形检出率
降低严重缺陷儿出生
提高我国人口素质的目的
三级超声检查应在具有产前诊断资格的医院
由取得产前超声诊断资格的超声医师
进行检查
检查所使用的仪器要求分辨力高
图像清晰
四级超声检查
通常在前3层次的检查基础上开展
针对性地对某一特殊要求或目的进行详细检查
胎儿超声心动图检查属此范畴
产前超声检查的时机
孕妇在怀孕期间一般需要接受5次超声检查
早孕期超声检查
11到14周超声检查
20到24周超声检查
28到32周超声检查以及临产前超声检查
早孕期超声检查时间为妊娠6到8周
检查内容有确定妊娠囊位置
除外异位妊娠
确定妊娠囊数目
确定有无胚芽及原始心管搏动
测量孕囊及胚芽大小以评估孕周
检查孕妇子宫及附件是否可见异常包块及肿瘤
11周到14周超声检查内容包括
确定胎儿数目及绒毛膜性
对胎儿的头臀长 颈项透明层进行测量
用以筛查胎儿染色体异常
对胎儿进行早孕期结构畸形筛查
妊娠20到24周超声检查内容
有胎儿生物学测量
对包括胎儿头颅 颜面 脊柱 胸腔
腹腔及四肢在内的各解剖结构的畸形筛查
胎盘 脐带 羊水等附属物的检查
妊娠28到32周超声检查内容有
通过胎儿生物学测量评估胎儿生长发育情况
对胎儿脑积水
肿瘤等妊娠中晚期才出现的异常进行筛查
检查胎盘 脐带 羊水等附属物
临产前超声检查的时间
一般在妊娠37到40周
检查内容包括评估胎儿胎位和大小
评估胎盘分级以及羊水量
多普勒超声测量胎儿脐动脉血流频谱
评价胎盘储备功能
测量宫颈长度 预测分娩发动时间
